Quality Starts & QS%

If a 'Quality Start' is a game in which the starter pitches 6 or more innings and allows 3 or fewer earned runs then why are there pitchers with more quality starts than games started?

If 'Quality Start Percentage' is indicates the percentage of all the games started that resulted in a 'Quality Start' then why do some pitchers have a 'QS%' greater than 100%?

I've seen that too. Pretty sure that's a bug.
